New UDS+ Reporting Requirements Announced for Health Centers
Health centers have long been required to submit a Uniform Data System (UDS) report to the Health Resources and Services Administration (HRSA) by February 15 each year. However, AAFCPAs would like to ensure FQHC clients are aware HRSA introduced a new requirement, the UDS+ filing due by April 30, 2025, for calendar year 2024 data.
The UDS+ filing represents a significant shift in how health centers report data. Instead of the traditional aggregate, tabular format, health centers will now submit de-identified patient-level data using Fast Healthcare Interoperability Resources (FHIR) standards. This change aims to provide deeper insights into patient outcomes, improving quality across the Health Center Program.
UDS+ 2024 Submission Requirements
For the 2024 reporting year, health centers must at a minimum submit UDS+ data for their medical patients. Those capable of doing so may report data for their entire patient population.
The following demographic tables are required.
- Patients by ZIP Code
- Table 3A: Patients by Age and by Sex Assigned at Birth
- Table 3B: Demographic Characteristics
- Table 4: Selected Patient Characteristics
Additionally, health centers must submit at least one electronic Clinical Quality Measure (eCQM) from one of the following clinical tables.
Table 6B: Quality of Care Measures
- Cervical Cancer Screening
- Breast Cancer Screening
- Colorectal Cancer Screening
Table 7: Health Outcomes and Disparities
- Controlling High Blood Pressure [HRSA recommended]
- Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9.0%)
Health centers must submit aggregated legacy UDS data through HRSA’s Electronic Handbooks, the official submission of record for the 2024 reporting year. This submission is due by February 15, 2025.
The move towards UDS+ reflects HRSA’s commitment to enhancing the quality of care provided by health centers and ensuring that the data collected is actionable and reflective of patient outcomes. The transition, backed by the support of nearly 300 health centers during the CY 2023 UDS+ voluntary period, is an important step toward full-scale UDS+ implementation in the coming years.
